Meine Katze liegt neben mir, sie hat Blähungen. Das heißt bei ihr – sie hat einmal gepupst. Die pupst sonst nie! Was mein Sohn aktuell zum schießen findet (er entwickelt langsam ein Faible für Fäkalien), bringt mich zum Nachdenken. Wie schnell das eben ging, aus einem einzigen Pups direkt Blähungen zu schlussfolgern. Ab wann sind wir krank, oder: Was bedeutet „Sicherheit“?
Die Unterscheidung zwischen einem auffälligen Screeningergebnis und einer Diagnose ist vielen Schwangeren bestimmt nicht klar, nehme ich an. Denn das Kreuzchen bei „Risikoschwangerschaft“ ist schnell gesetzt. Auch, wenn die Screeningergebnisse nur leicht von den geforderten Werten abweichen und auch, wenn noch gar keine Diagnose möglich war
Laut Definition ist eine Diagnose eine „bewertete Zusammenfassung der Symptome und Befunde eines Patienten, die in der Feststellung und Benennung der zugrundeliegenden Krankheit gipfelt. Dabei fließen auch Normalbefunde ein.“ Können Ärzt*innen also eine Diagnose stellen, wenn sie nur ein einziges Screeningergebnis haben? Nein. Deswegen folgen nach dem auffälligen Ergebnis weitere Untersuchungen oder Beobachtungen. Natürlich nicht, ohne die Patient*in vorher über alle Eventualitäten aufzuklären. Liegen dann mehr Ergebnisse vor, unternehmen sie weitere Schritte, dann ist eine Diagnose möglich. Doch den Stempel hat die Frau trotzdem schon früher. Wenn nicht im Mutterpass, dann im Kopf.
Was passiert in der Zwischenzeit?
Ärzt*innen sind nur Menschen und die einen haben mehr, die anderen weniger Einfühlungsvermögen. Was aber alle definitiv nicht haben ist Zeit. Mehr als 10 Minuten im Sprechzimmer sind absoluter Luxus und da ich nicht privat versichert bin, weiß ich nicht, in wieviel Aufklärungsgenuss die Privatpatient*innen so kommen. Fachärzt*innen sind spezialisiert und setzen sich mit ihrem Fach hoffentlich auseinander, jeden Tag. Manche sind bereits abgestumpft, manche haben keine Lust mehr auf Fortbildungen, vielen fehlt das Verständnis dafür, dass ihre in fünf Minuten runtergerasselten Aufzählungen aller möglichen Risiken niemanden wirklich aufklären. Das ist einfach nur beunruhigend.
Zwischen dem ersten Gespräch nach ein paar Auffälligkeiten im Ultraschall oder den vorliegenden Laborergebnissen und dem nächsten Termin zu Abklärung liegen manchmal nur Tage, manchmal Wochen. Wie fühlt sich diese Zeit an? Selbst in ganz harmlosen Fällen richtig scheiße. Denn je nachdem, in welchem Tonfall der Arzt mit mir gesprochen hat, wie sie mich gemustert hat, wie schnell sie sprachen oder natürlich um was es sich eigentlich handelte, ließen mich die Aussagen ratlos bis ängstlich zurück. Ich spreche nur von mir und vermute nur, wie vielen anderen es genauso geht. Nicht jede*r Patient*in oder Schwangere schreibt seine oder ihre Sorgen ins Internet. Und möglicherweise ist das Internet eher ein Abbild von Sorgen, als von den ereignislosen, glücklichen Tagen.
Ich habe kein Problem mit Vorsichtsmaßnahmen
Vorsorgentersuchungen sind wichtig, es gibt Komplikationen die früh erkannt gut behandelt werden können. Es ist ganz wunderbar, zu was die Medizin heute fähig ist und wie niedrig die Sterberaten von Müttern und Kindern in Industrienationen heute sind. Auch unerkannte Schwangerschaftsdiabetes hat so viele vermeidbare Risiken in petto. Ein Glück, dass die Krankenkassen den oGTT für alle Frauen vorsehen. Das war vor zehn Jahren noch nicht so. Auf die Weise erkennen Ärzt*innen tatsächlich mehr Fälle von Diabetes in der Schwangerschaft. Es ist nicht mehr beschränkt auf die Frauen, die sich den zusätzlichen Test leisten können.
Ich bin selbst tatsächlich sehr sicherheitsbedürftig. Angesichts der Tatsache, dass ich eine Hausgeburt plane, können einige Gyäkolog*innen und Mütter (und Väter und alle, die es eigentlich nichts angeht) nur mit dem Kopf schütteln. Passt das zusammen? Ich finde, ja. Sicherheit entsteht nicht durch noch mehr Untersuchungen und Auswertungen von noch mehr Laborwerten und Ultraschalluntersuchungen. Wenn bei allen Untersuchungen und Messungen immer alles tip und top ist, vielleicht stellt sich dann bei einigen ein gutes Gefühl ein. Aber ganz ehrlich: wie oft bleibt im Hinterkopf ein „und was, wenn“? Wer viel misst, misst viel Mist, heißt es doch so abgedroschen wie wahr.
Je mehr Menschen ich frage, desto mehr Meinungen bekomme ich.
Das fühlt sich dann für mich nicht mehr nach Sicherheit an. Sicher ist das auch eine Typfrage, manch eine braucht viele Ultraschalluntersuchungen, um sich sicher zu fühlen. Aber was heißt denn Sicherheit, wenn das beruhigende Gefühl nur eine Woche anhält? Ich bin in so einem Setting alles andere als eine Schwangere, die in sich ruht und guter Hoffnung ist.
Ein ausgerutschter Nüchternblutzuckerwert reichte aus, um mich für eine Woche völlig durchdrehen zu lassen.
Ich habe ja bereits erwähnt, dass ich meinen HOMA-Index zu Beginn der Schwangerschaft habe messen lassen. Damit wollte ich frühzeitig reagieren, falls meine Werte auf eine Insulinresistenz hinweisen. Je früher ich mit Ernährungsumstellung gegensteuere, desto besser ist das für das Baby und für mich. Dachte ich. In Wahrheit hat mich die Messerei verrückt gemacht. Obwohl sich nach einigen Wochen herausstellte, dass bei der Blutabnahme ein Fehler passiert war und ich tatsächlich viel bessere Werte habe als angenommen, hatten mich die plötzlich guten Werte nicht mehr beruhigt.
Da die Überweisung zum Diabetologen bereits parat lag, machte ich einen Termin. Denn ich wollte auf Nummer sicher gehen. Weil der aber nichts vom HOMA wissen wollte und lieber selbst auf Nummer sicher gehen wollte (ach), machte ich den 75er oGTT. Der Nüchternwert war etwas über der erlaubten Grenze und zack: Gestationsdiabetes, ab damit in den Mutterpass. 10 Tage Blutzuckertagesprofil mit täglich sechs Messungen und von der Frauenärztin eine Überweisung zur Feindiagnostik. Nicht wegen des Hämatoms, das anfangs insgesamt drei Ärzt*innen für eine Plazenta Praevia gehalten hatten. Sondern weil das Kind möglicherweise zu groß sein könnte.
Nachdem ich die Gestationsdiabetes-Leitlinien zwei Mal durchgelesen hatte und dort las, dass frühzeitige Screenings eine unnötige Belastung für die Schwangere und das Gesundheitssystem darstellen, musste ich lachen. Denn in einer großen Studie aus China hatten nur 1/3 aller Schwangeren mit erhöhten Nüchternblutzuckerwerten in der Frühschwangerschaft beim oGTT zwischen der 25. Und 29. Schwangerschaftswoche einen Wert über 5,1 mmol/l. Eine genaue Darstellung der Screeningergebnisse in der besprochenen Studie gibt es in der Leitlinie Gestationsdiabetes mellitus (GDM) in Kapitel 6.2 ff.
Ich habe mich freiwillig dem Prozedere unterzogen und alles was ich davon hatte, war eine Panikattacke im Wartezimmer
Nie war ich dankbarer für die Herzkohärenz-Atmung, die ich in meinem Anti-Stress-Kurs für werdende Mütter kennengelernt habe. Mit Blick auf meine protokollierten Werte sagte mein Diabetologe dann nur: „Ja, so sehen die Werte von einer stoffwechselgesunden Person aus!“
Nun soll ich weitere sechs Wochen punktuell meinen Blutzucker kontrollieren, denn einfach so die Diagnose zurücknehmen wollte er dann auch wieder nicht. Gut, die Richtlinien habe ich ja gelesen. Der Mensch hält sich streng daran. Er fand zwar, dass das Baby schon ein großes Exemplar sei für sein Alter, aber dass es ja noch im Rahmen sei und damit war auch das Thema vom Tisch.
Jetzt kann es mir doch gut gehen, oder?
Die Plazenta hat sich an die Hinterwand verzogen, das Hämatom, wegen dem ich mich acht lange Wochen streng schonen sollte: weg. Und die Insulinresistenz: Im Prinzip nicht existent. Ich bin jetzt in der 22. Schwangerschaftswoche. Wir sind über den Berg gerollt. Mein Bauch wächst und das Baby strampelt und boxt und tritt und hat Schluckauf. Mein Sohn schmiegt sich an meinen Bauch und bietet dem Baby manchmal etwas zu trinken an. Ich habe mich gegen die Feindiagnostik entschieden, weil ich diese „Interventionen […] mit zu viel Belastung für die Frau[en] und das Gesundheitssystem verbunden [hält].“ (Aus dem Zusammenhang gerissenes Zitat aus den Gestationsdiabetes-Leitlinien.)
So fühle ich mich sicherer:
zuhause. Bei meiner Familie. Mit Menschen, denen ich vertraue in einer Atmosphäre, die Geborgenheit ausstrahlt. Mit Gerüchen, die ich kenne und mag. Hier senkt sich mein Stresslevel. Und hier kann ich entscheiden: Wenn es nicht anders geht, etwas passiert, dann fahren wir ins Krankenhaus. Was eine Geburt sicher macht ist individuell und ein weites Feld. Der Teufelskreis durch Stress, der den Blutzucker in die Höhe treibt und dadurch auffällige Werte, was wiederum Stress hervorruft ist mir zu viel. Und völlig unverhältnismäßig, angesichts der Tatsache, dass meine Werte bisher alle okay waren. Ich stelle mir vor, was Ärzt*innen bei der Feindiagnostik außerdem noch alles ins kritische Licht der tausend Meinungen rücken. Was das für Kontrollen nach sich zieht, wie viele Meinungen und Ratschläge ich dann bekomme und was ich dann für Richtlinien studiere: Da schnürt sich wieder alles zu. Ich spüre, wie die Panik hochkriecht und mein Bauch hart wird. Die Annahme, es könnte ja alles auch einfach gut sein, gilt nicht. Von hundertprozentiger Sicherheit kann niemand je sprechen. Und das zu akzeptieren ist für mich wichtiger. Dazu brauche ich Vertrauen in meinen Körper und mich und vielleicht in so etwas wie Schicksal.
